Chronic Disease Management
In response to changing health care trends, the Riverbrook Regional YMCA and Western Connecticut Health Network is teaming up to deliver community-based health and wellness services in innovative new ways rooted in a common commitment to prevention and combating chronic disease.
YMCA members who visit the Wilton Family YMCA branch can take advantage of health screenings and risk assessments provided by a Registered Nurse Health Coach from Norwalk Hospital that will be placed in the Y facility. To accommodate the growing demand of wellness programs, the Health Coach will provide clinical wellness coaching services and health education workshops on weight loss, nutrition, diabetes, smoking cessation, healthy back, healthy bones, stress management, spirit/mind health, fitness and more.

LIVESTRONG at the YMCA
Our YMCA is proud to offer LIVESTRONG at the YMCA to help cancer survivors feel alive again! This 12-week program is designed to help survivors overcome the effects of fatigue and deconditioning from treatment and the disease. The program is for adult survivors from the point of diagnosis through the balance of life and includes a family membership during the 12-week program. Our YMCA is accepts registrations for upcoming sessions at anytime throughout the year. A physician referral is necessary to join the program.
Participants do not need to be YMCA members – so please share this with anyone you think might be interested. Prior to beginning the program, all participants will be asked meet with a LIVESTRONG at the YMCA specialist for an intake appointment. The intent of the intake is to properly review each survivor’s medical needs and cancer history to create a personal and appropriate wellness plan. Participants work with Y staff trained in supportive cancer care to safely achieve their goals such as building muscle mass and strength; increasing flexibility and endurance; and improving confidence and self-esteem. By focusing on the whole person and not the disease, LIVESTRONG at the YMCA is helping people move beyond cancer in spirit, mind and body.

The Riverbrook Regional YMCA is one of a few selected YMCA’s to receive a grant for the “Livestrong at School” program. Livestrong at School is a K-12 curriculum is designed to raise cancer awareness in school age children., LIVESTRONG challenges advocates, educators, parents and others to get students involved in the fight against cancer. Education is the key to combating the disease. Through this program the Riverbrook Regional YMCA Livestrong staff helps children understand the disease and helps support their efforts to combat the disease.
Parkinson’s Exercise Program
To date, despite decades of intensive study, the causes of Parkinson’s remain unknown. Many experts think that the disease is caused by a combination of genetic and environmental factors, which may vary from person to person.
In some people, genetic factors may play a role; in others, illness, an environmental toxin or other event may contribute to PD. Scientists have identified aging as an important risk factor; there is a two to four percent risk for Parkinson’s among people over age 60, compared with one to two percent in the general population.
• As many as one million Americans live with Parkinson’s disease, which is more than the combined number of people diagnosed with multiple sclerosis, muscular dystrophy and Lou Gehrig’s disease.
• Approximately 60,000 Americans are diagnosed with Parkinson’s disease each year, and this number does not reflect the thousands of cases that go undetected.
• An estimated seven to 10 million people worldwide are living with Parkinson’s disease.
• Incidence of Parkinson’s increases with age, but an estimated four percent of people with PD are diagnosed before the age of 50.
• Men are one and a half times more likely to have Parkinson’s than women
• Medication costs for an individual person with PD average $2,500 a year, and therapeutic surgery can cost up to $100,000 dollars per patient.
The Riverbrook Regional YMCA offers a Parkinson Exercise program, based on the “Delay the Disease” program protocol. “Delay the Disease” was developed in response to the growing number of individuals diagnosed with the disease looking to regain functional strength and resume their active daily living routines prior to their diagnosis. The 12 week program gives anyone with the disease the opportunity to work with a certified Parkinson Exercise specialist to increase their muscular strength, cardiovascular fitness and flexibility .Participants work in in small groups to improve gait, balance speech and motor skills. A pre and post assessment is given to all participants. Medical clearance is required in order to participate.

“Freedom From Smoking”
The American Lung Association’s Freedom From Smoking® (FFS) program is for adults who are ready to quit smoking. Because most people know that smoking is dangerous to their health, the program focuses almost exclusively on how to quit, not why to quit. The Riverbrook Regional YMCA is a provider of this program. Day and evening classes are currently forming both on-site at the YMCA and at various off-site locations..
How the Program Works
The Freedom From Smoking® program consists of eight sessions. It is delivered by a Lung Association-trained facilitator in a small-group setting (usually eight to 10 people), so participants receive personalized attention. At the same time, individuals benefit from the support of their peers, who are going through the same stages at the same time. The curriculum includes the latest research about nicotine replacement therapy (gum, inhalers, patches, lozenges and nasal spray) and other smoking cessation medications such as Zyban® and Chantix®.
Each session uses techniques based on pharmacological and psychological principles and methods designed to help smokers gain control over their behavior. Because no single cessation technique is effective for all smokers, the program includes a comprehensive variety of evidence-based cessation techniques. Group classes are currently forming.

Prediabetes is a potentially reversible condition that often leads to diabetes, and 79 million people in the United States are estimated to have it. If you have been diagnosed with prediabetes, or believe you may be at risk for developing the disease, the YMCA’s Diabetes Prevention Program can help you develop a healthier lifestyle.
The YMCA’s Diabetes Prevention Program gets to the root of the diabetes epidemic by helping those at high risk of developing the disease make lifestyle changes. The program focuses on healthy eating, increased physical activity, and maintaining a healthy weight.
Based on effective efforts researched by the National Institutes of Health and the Centers for Disease Control and Prevention, the YMCA’s Diabetes Prevention Program will help you learn about and adopt the healthy eating and physical activity habits that have been proven to reduce the risk of developing type 2 diabetes. Through the program you will receive support and encouragement from both a trained lifestyle coach and fellow classmates as you develop a plan for improving and maintaining your overall well-being.
About the Program
As a leading nonprofit for strengthening community through youth development, healthy living and social responsibility, the Y believes that all people should be able to live life to its fullest, healthiest potential. In the YMCA’s Diabetes Prevention Program a trained lifestyle coach will introduce topics in a small classroom setting and encourage participants as they explore how healthy eating, physical activity and behavior changes can benefit their health.
The 12-month group-based program consists of 16 core sessions, followed by monthly maintenance sessions led by a trained lifestyle coach who facilitates a small group of people with similar goals.
Discuss topics such as healthy eating, increasing physical activity, reducing stress, problem solving, and much more. Stay motivated to maintain progress towards program goals with monthly maintenance sessions.

The Riverbrook Regional YMCA is offering the YMCA Blood Pressure Management Program a program designed to help participants with hypertension lower their blood pressure through an evidence-based program that combine four elements:

and Personalized Support

Research shows that the simple process of checking and recording your blood pressure at least twice a month over four months may lower blood pressure in people with high blood pressure.
For a small investment of time, participants receive:
- A simple, evidence-based program that may lower blood pressure
- Personalized support from a Y Healthy Heart Ambassador
- Tips for maintaining cardiovascular health
- Access to select Y group physical activity classes
- Monthly nutrition education seminarsTo qualify for participation in this Program:
- Must be at least 18 years old
- Must be diagnosed with high blood pressure
- Must not have experienced a recent cardiac event
- Must not have Atrial fibrillation or other arrhythmias
- Must not be at risk for lymphedema

Enhanced Fitness Classes
The Riverbrook Regional YMCA will be offering a new program for those suffering from arthritis or other chronic pain diseases. The enhanced fitness program fosters improved health and well-being in members, including enhanced balance and flexibility, leading to stronger, healthier, more confident individuals. Enhance Fitness focuses on dynamic cardiovascular exercise, strength training, balance, and flexibility—everything older adults need to maintain health and function as they age. Free to Y Members!
In a typical class participants will experience:
* A 5-minute warm-up to get the blood flowing to the muscles
* A 20-minute aerobic workout that gets participants moving or
a walking workout to lively music that the class chooses.
*A 5-minute cool-down
*A 10-minute stretching workout to keep the muscles flexible
*Balance exercises throughout the class.
Obesity effects 17% of all children and adolescents in the United States-this is triple the rate from one generation ago. Today – one in 6 children are living with obesity and one in three are overweight.
Healthy Weight and Your Child (HWYC) is an evidenced based program that aims to empower children and families through education, healthy eating and physical activity to help families learn skills and behaviors to live a healthier lifestyle.
The yearlong program is designed for children ages 7—13 years of age with a body mass index of the 95th percentile or higher.
Children must have clearance from a health care provider or school nurse in order to participate.
A parent must attend all sessions. The program includes 16 weekly sessions followed by post core sessions for the remainder of the year.
